We'll be in touch promptly to provide you with pledge forms, news about special awards and prizes, and hints to help you collect more pledges.

In clicking on the Submit button below, I acknowledge that I understand the intent thereof, and I hereby agree and absolve and hold harmless The Sunshine Foundation of Canada, corporate sponsors, co-operating organizations and any other parties connected with this event in any way, singly or collectively, from and against any blame and liability for any injury, misadventure, harm, loss, inconvenience, or damage hereby suffered or sustained as a result of participation in the 2004 Walk for Kids' Dreams or any activities associated therewith. I hereby consent to and permit emergency treatment in the event of injury or illness. I also give full permission for use of my name and/or photograph in connection with this event.

 

 

Sunshine Dreams for Kids makes dreams come true for children ages 3 to 19 who are challenged by severe physical disabilities or life-threatening illnesses.
